Important Forms and Payments

All forms and lab fee payments are due THURSDAY, JUNE 11TH, 2009.

PAPA Information Sheet and Statement of Agreement

The Paliku Academy of Performing Arts requires that each student enrolled have two forms on file: an information sheet and a statement of agreement.  The information sheet contains important data regarding medical history, emergency contacts and health care information.  This sheet also gives parents, family and friends an opportunity to volunteer in areas we need assistance in.  The second sheet is basically an agreement both the parents and students sign that discusses the issue of behavior.  This will assist PAPA instructors and staff when dealing with behavior issues.  If we do not receive these forms, your child will not be able to participate in auditions.  Please have your child bring these forms on the first day of sessions.

Lab Fee

There is a $100.00 Lab Fee for every child enrolled.  This charge is to pay for any costumes, rehearsal materials, etc. for your child.  If for any reason you are unable to pay by the deadline, you need to speak to the Director prior to the due date to set-up a payment plan.
